Patient Support after Discharge
We wish to ensure that our patients continue to receive medical care and support after they have been discharged from hospital. At the Hospital SanaFontis preparations for discharging a patient after hospital treatment start early in the form of counselling interviews in which we involve the patient’s family as well. Throughout the patient’s hospital stay we help him to acquire the particular skills he will need to take care of himself after returning home. While doing so we take into account the social, economic and work situation the patient will find himself in after his discharge from hospital.
We define qualified patient support as follows:

- Empowering patients to fulfil tasks and solve problems
- Individualised and specific preparation of the discharge from hospital
To ensure continuous patient support we are aiming for a smooth collaboration between the patient, his family, his family doctor and the hospital physicians. This collaboration includes:
- Initiation of rehabilitation
- Preparation of home care
- Arranging a nursing service
- Preparation of short-term or long-term admission into a nursing facility
